Letter of Recommendation Form - Outstanding Graduate Student Teaching Award 2018-19

This form is only to be completed by an applicable faculty member. The individual graduate student requesting this recommendation is applying for the 2018-19 Outstanding Graduate Student Teaching Award. It would be helpful in our selection process if you would provide us with your assessment of this applicant using this form.

Deadline for Letter of Recommendation Form = Friday, April 5, 2019

This letter is considered confidential and completed by the individual who supervised the course for which the candidate was nominated. If the student is the instructor of record, the Supporting Letter of Recommendation should be from the department chair, graduate program chair, or a designated faculty member.

The designated faculty member will be sent a link via email for access to this form. Emails will be sent after January 1, 2019.
